Thank you for your email.
As an international student, we do ask for official documentations for your immunization records. We also ask students to complete an International Student Health History and Immunization Form, which can be found on our website at: Forms | Student Health Services | Oregon State University. I have also attached the form with this email for your reference.
You may submit documentations of your immunization records and health forms by fax: 541-737-9665; by mail: OSU Immunization Coordinator, 201 Plageman Building, Corvallis, OR 97331; or by email (scanned) to:immunizations@oregonstate.edu. If you are not able to send in your immunization records, you will be required to receive all of the required immunizations when you arrive.
Please see below for the required immunizations.
o INTERNATIONAL STUDENT IMMUNIZATION REQUIREMENTS
o ALL INTERNATIONAL STUDENTS:
o Documentation must be submitted to Student Health Services in addition to the online form in order to satisfy the requirement. Please fax, mail or email a copy of your immunization records showing
o 2 doses of MMR (Measles, Mumps, Rubella).
o 2 doses of Varicella (Chicken Pox), or the date of disease.
o 3 doses of Hepatitis B, or the date of disease
o 1 dose of Tdap (Tetanus, Diphtheria, Pertussis), since 2005.
o ALL STUDENTS HEALTH FORM REQUIREMENTS
o Medical History Forms
o Tuberculosis Screening Form
All requirements must be complete or a hold will be placed on your account.

7 " وانا معيد بجامعة الملك خالد وحصلت على بعثه وكان اختياري على معهد( into osu )
والحمد الله سوف تبد دراستي في 3/1/1440 اي بعد شهر وشوي وحبية استفسر عن السكن في المدينه كيف اسكان وايهم افضل السكن مع العايلات أو سكان الجامعة من كل النواحي من حيث اتكلفه طريقة العيش خاصتن واني اريد تعلم اللغه بشكل جاد يعني اريد المعلومات بي التفصيل الممل
